Financing Your Care
In our office, we offer high quality care and payment options that work for all of our patients. We are participating providers for the following Insurance Plans. We also accept many other policies not listed here. There are many different dental policies. It is always best to check with your insurance carrier to verify if we are on their provider list. The phone number for your insurance carrier can be found on the back of your insurance card. Please contact our office at 320-629-2789 if you have questions concerning your insurance coverage or payment options.

We have established the following payment guidelines to reduce the paperwork involved in billing, while maintaining a superior level of professional care.
- Payment in full is expected and due on the day treatment is rendered.
- If you are covered under an insurance plan, the estimated portion that the insurance will not cover is expected and due on the day treatment is rendered.
- Payment can be made using Cash, Check, or your Visa/MasterCard/HSA/Flex card.
- Third party extended payment financing is available upon approval with Care Credit.
If an account is not paid in full within 60 days of the date of service, the account will be evaluated by our financial team. Accounts outstanding more than 60 days will bear interest of 1 1/2% per month or 18% per annum, and may be subject to collection agency fees, legal fees, and other expenses incurred in the collection of your account. Because dental laboratories require prompt payment from us, a down payment from you is necessary to offset our “upfront” expenses. This down payment is to be paid on the day of the impression for extensive treatment. A fee of $25 is charged for returned checks, payable by cash or credit card. This will be applied to your account in addition to the insufficient funds amount.
Please note: Payment is due at the time of service.
As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within two to four weeks. Any remaining cost is your responsibility.
